Affiliate Columns: allow Person Name, not just Affiliate Name

It's confusing to have "Name" be the column heading because it's actually Affiliate Name... not the Person Name. Some of my affiliates like to use a word or a company name and I need to be able to easily identify WHO that affiliate is.
- Please add the ability to include Person Name as one of the columns.
- Rename "Name" to "Affiliate Name" for clarity.
